Freshservice - Service Request - New Application Request

The first step is to log into FreshService using SSO 2. Select "Request a Service" 3. Select "Requests for IT" 4. Select "New Application Request Form" 5. Select the "Name of 3rd Party Application" text box. Enter the name of the application you wish to request. 6. Select the "Is this a stand-alone app or a Microsoft Teams app?" dropdown box. Then indicate whether it is a Microsoft Teams Application or a Stand Alone application. 7. Select the "If Possible, please include the URL for the Publisher Site" textbox. Then enter the URL for the publisher site if you have it available. This will make investigation of the application easier if there are many apps with the same name. 8. Select the "What is the business purpose of this app?" Textbox. Enter a description of your groups need for the application. 9. Select the "What individuals need Access to this App?" textbox. Please enter the names or the group name of the individuals that need access to this application. 10. Select the "Does this App require a license?" drop down menu. 11. Select whether the application requires a license to the best of your knowledge. 12. Select "Place Request" 13. Confirm all details are correct. Then select Confirm. That's it! You're Done!

Freshservice - How to Report an Issue
If you need to report an IT issue please do the following 1. Go to https://support.ksmcpa.com/support/home and please use your network credentials if prompted. 2. Select Report an Issue 3. As you enter a subject in the subject line, review the Suggested Articles on the right side to research the issue you might be having. Select the article to review a possible solution: 4. Please fill out the form with as much detail as possible, including screenshots and error messages. Use the following as a guide: Client Number and Name Year (if applicable) Application you are using Screenshots of error messages 5. After completing the form, please hit Submit. An IT Support Technician will contact you after reviewing the issue
